Planning and Development

Our health system's Planning and Development Department is responsible for is the comprehensive process of supporting and overseeingall activities related to a grant. The process includes managing the major steps of a grant lifecycle: Pre-Award | Award | Post-Award. Grants managers oversee various components of a grant lifecycle in varying detail.

Additionally, this team supports our Winnebago Health Foundation which is the 501c3 nonprofit arm of WCHS.

  • All staff wishing to submit a grant proposal must complete this form, as it enables leadership to stay informed and give approval for potential grant projects.

    Completion of the form is REQUIRED BEFORE you begin work on a grant application. This form will also help ensure that you have the necessary approvals for committing matching funds and will help you and the Planning & Development Department if you are funded.

    Please complete this form, by tabbing through the open fields, and send to the Planning & Development Department. A completed Grant Application Pre-Approval Form is required for the appropriate level of approval and signature for your grant proposal or grant contract.

  • Renewal grants require a Grant Application Pre-Approval Form

    • if the project involves committing WCHS funds/services that have not already been approved

    • if the project scope has changed; and/or

    • if a new contract must be signed by the Board of Directors, Governing Body, or another Executive.

  • CLICK HERE TO ENROLL in Payroll Deduction by completing this form is the easiest way to contribute. Payroll deduction allows you to determine any amount to come out of each paycheck on a recurring basis.

    • You can donate or set up payroll deduction at any point throughout the year.

    • There is no maximum amount you can donate, but there is a minimum of $1 per check. Your gift per paycheck can add up to a lot over time.

    • Payroll deductions are made over the 26 pay periods within each year. We are not able to accommodate one-time donations through payroll at this time.
